Medical Forms & Medication Planning for Summer 2026

Getting ready for camp is exciting, and we know it also comes with a checklist. One of the most important steps in preparing for a smooth, safe, and healthy summer at Beber Camp is completing all required medical forms and medication planning on time.

Our Health Center team works incredibly hard to ensure every camper receives excellent care, and that starts with accurate information from families. Below is a clear guide to what’s required, what’s due, and how to plan if your child takes medication.

📅 Medical Forms Deadline: April 1

All medical forms must be completed and uploaded in CampInTouch by April 1.

Required Medical Forms

☑️Health History Form
  • All medications must be listed, including:
    • Prescription (RX)
    • Over-the-counter (OTC)
    • Vitamins and supplements
  • This includes any medication sent to camp or through Pack My Rx
  • Please list all medications in the prescriptions section of the Health History Form
  • If you make any changes after April 1, you must email trudy@bebercamp.com
  • Detailed instructions are available on our website under:

☑️Immunization Records
  • Upload a record from your doctor, state, or school
  • We only require immunizations current as of April 1
  • New for 2026:
    If your child attended Beber Camp last summer, you do not need to upload a new immunization record unless your child received an additional immunization this year
  • If new immunizations occur after April 1, no update is needed this year
  • A list of required immunizations can be found on our website under:
    Current Families → Getting Ready for Camp → Health & Wellness
☑️Insurance Cards
  • Upload front and back of all insurance cards
  • This is required every year, even if the insurance has not changed
  • Be sure all insurance details are also entered in the Health History Form
  • If you have a separate RX insurance card, please upload that as well
☑️Medication Authorization Form
  • This form is ONLY for medications not coming from Pack My Rx
  • It is required for certain exempt medications (outlined below)
  • The form must be uploaded to CampInTouch

💊Medication at Camp: What Families Need to Know

Sending medication to camp requires some advance planning. If you have any questions at any point, please reach out to trudy@bebercamp.com. We are here to help.

A Few Important Guidelines

  • We strongly discourage “medication vacations” at camp
  • Camp is not the time to:
    • Start a new medication
    • Change a dosage
    • Discontinue a medication your child takes regularly
  • If a change is unavoidable, please contact Trudy before camp

Please note:

  • You do not need to send medications like Tylenol, ibuprofen, or Benadryl unless taken daily
  • We stock basic generic medications for occasional needs
  • We do not stock:
    • Naproxen (Aleve)
    • Melatonin
    • Dramamine / Bonine
    • Lactase (Lactaid)

⚕️Pack My Rx: Required for Most Daily Medications

To ensure the highest standard of care in our Health Center:

All campers who take DAILY pill medications or supplements, including prescription and OTC medications, vitamins, and melatonin, must enroll in Pack My Rx, our required pharmacy service.

Pack My Rx:

  • Packages medications in individual, clearly labeled pouches
  • Includes camper name, medication, date, and time of administration
  • Helps reduce errors and keeps medication administration consistent and safe

If Pack My Rx cannot accept your insurance or fill a medication, we will work with you on next steps. If this happens, please contact trudy@bebercamp.com immediately.

🆕New for Summer 2026: Melatonin Update

  • Please list melatonin as “as needed” with Pack My Rx
  • It will be packaged separately
  • Campers may come to the Health Center nightly if they choose
  • We will not require nightly use or chase campers who do not come

📝A Note About Medication Forms

  • If your child takes only ONE daily OTC pill (such as Zyrtec or a pill vitamin), an alternate option may be available
  • You must contact trudy@bebercamp.com at least 60 days before camp for instructions

We do not accept:

  • Gummies
  • Liquids
  • Dissolvable tablets (such as Claritin Redi-Tabs)

Please switch to chewables before camp so your child is comfortable taking them. There are no exceptions for gummies.

⛔Medications Exempt from Pack My Rx

The following medications do not have to come through Pack My Rx:

  • Accutane
  • “As needed” medications (e.g., Imitrex, Zofran)
  • Creams
  • Eye drops and ear drops
  • Injectable medications (epi-pens, insulin, etc.)
  • Inhalers
  • Nose sprays
  • Oral contraceptive pills
  • Powders (such as Miralax)
  • Lactase (contact Trudy for instructions)

Most exempt medications must arrive at camp at least four days before the session begins. Only Accutane, current oral contraceptive packs, one emergency inhaler, and one set of epi-pens do not need to arrive in advance.

Some medications cannot be sent through Pack My Rx at all (including oral contraceptives, Accutane, insulin, and growth hormone injections).
